2008–2009 SMRE Registration Instructions

General Information

2008–2009 Sunday Morning Religious Education:

• Is for children in Preschool (age 4) through 8th Grade,
• First day of class is Sunday, September 7, 2008; and
• Classes are held Sunday mornings from 9:00–10:15 a.m. in the OLG school.

Sign-up Instructions

Instructions are the following:

• Print the two-page Family Registration Form
  • First section, fill in your family information.
  • Second section
    • List your child(ren) that you wish to enroll and re–enroll in SMRE.
    • Make sure to list the grade level for each child.
  • "Date" and "sign" the Family Registration Form at the bottom of the first page.
  • On the second page you will need to complete the emergency information section regarding your children during SMRE class time.

• Print the two-page Student Information Form
• This form should be filled out:
  • for each new student you are registering, or
  • for any changes regarding your child who attended SMRE last year.
• Note:  The Student Information Form does not need to be filled out if there are NO changes in your child's information.

Fees and Payment Procedures

Enrollment fees for children Pre-school age 4 through Grade 8 are as follows:

• One Child  —  $60.00
• Two Children  —  $80.00
• Three or More Children  —  $90.00

Note:  Pre-school child must be age four by June 1.

Note:   Make checks payable to Our Lady of the Greenwood Church. If you anticipate any difficulty in paying program fees, please contact us at (317) 888-2861, extension 236 to make alternate arrangements. We do not want fees to prevent any child from participating in religious education opportunities.
 
Be sure to return the Family Registration Form, Student Information Form, and payment to the parish office no later than August 31, 2008.
